Pursuant to TX House Bill 4224 Section 181.105 of the Texas Health and Safety Code, please be advised of the following:
A. HOW TO REQUEST ACCESS TO HEALTH CARE RECORDS FROM Spine and Sport Family Chiropractic:

- Requests for records shall be in writing. (include specific information to disclose and to whom it will be disclosed)
- A patient or other person, legally authorized to act on their behalf, shall sign the request.

- Name, identifying information, dates of service
- Allow 15 business days to fulfill the request
